How does Renagel work? What will it do for me?

Renagel is a medication that binds a substance known as phosphate. Phosphate is made up mostly of phosphorus, which is an important component of normal bone and cell metabolism.

The kidney helps keep the amount of phosphorus in the blood at a relatively constant level. As kidney function reduces, less and less phosphorus is removed from the body. High levels of phosphorus in the bloodstream can contribute to problems with the strength and structure of bones, among other things. This condition is known as renal osteodystrophy.

When taken with meals, Renagel prevents the absorption of phosphates through the intestines. It is used specifically to prevent high levels of phosphate (and therefore phosphorus) in the blood for people with severe kidney disease known as end stage renal disease (ESRD) who are using dialysis machines to rid their blood of toxins and waste.

Your doctor may choose to use Renagel for other conditions not listed here. If you're unsure why you are taking Renagel, please ask your doctor.

How should I use Renagel?

The dose of Renagel depends on the amount of phosphorus in the blood. The dose ranges from 6 capsules daily (2.4 g) to 12 capsules daily (4.8 g). The capsules should be taken immediately before or with meals and the capsules should not be bitten, chewed, or broken apart before swallowing.

Many things can affect the dose of medication that a person needs, such as body weight, other medical conditions, and other medications. If your doctor has recommended a dose different from the ones above, do not change the way that you are using the medication without talking to your doctor.

It is very important to take Renagel exactly as prescribed. If you miss a dose of this medicine, skip the missed dose and carry on with your regular schedule.

Renagel should be stored at room temperature and protected from heat, moisture, and light (do not keep it in the bathroom or the kitchen). Keep it out of reach of children.

Who should NOT take Renagel?

Renagel should not be used by anyone with:

low levels of phosphorus in their blood
bowel obstruction
an allergy to Renagel or any of the other ingredients

What side effects are possible with Renagel?

The side effects listed below are not experienced by everyone who takes Renagel. If you are concerned about side effects, discuss the risks and benefits of Renagel with your doctor.

The following side effects may go away as your body becomes used to the medicine; check with your doctor if they continue or become bothersome.

Are there any other precautions or warnings for Renagel?

Gastrointestinal disorders: The efficacy and safety of Renagel have not been established for people who:

have difficulty swallowing
have severe gastrointestinal motility disorders (disorders of movement in the gastrointestinal tract, which consists of the esophagus, stomach, and bowel)
have had major gastrointestinal surgery
have had appendicitis
have gastrointestinal bleeding
Renagel should be used with caution by these people.

Kidney disease: People with kidney disease may develop a condition called hypocalcemia (low calcium levels in the blood), and should have their calcium levels monitored. Levels of bicarbonate and chloride should also be monitored, since these substances may become depleted in the blood.

Pregnancy: There are no good studies of theis drug with pregnant women. It should be used during pregnancy only if potential benefits outweigh risks.

Breast-feeding: The safety of Renagel during breast-feeding has not been established. It should be used by nursing women only if expected benefits outweigh risks.
Appendicitis

Children: The safety and efficacy of Renagel have not been established in children.

What other drugs could interact with Renagel?

The potential exists for Renagel to bind to other drugs taken orally, preventing their absorption into the body. Despite the lack of documented interactions with Renagel, it is possible that the following drugs may be affected if taken with it:

antibiotics
medicines for an irregular heartbeat (antiarrhythmics)
levothyroxine and other thyroid hormones
seizure medications (anticonvulsants)
theophylline
warfarin
These potential interactions may be minimized if the other drugs are taken at least one hour before or three hours after taking Renagel .

Phosphate salts also interact with Renagel, since they increase the amount of phosphate in the body.

If you are taking any of these medications, speak with your doctor or pharmacist. Depending on your specific circumstances, your doctor may want you to:

stop taking one of the medications,
change one of the medications to another,
change how you are taking one or both of the medications, or
leave everything as is.
An interaction between two medications does not always mean that you must stop taking one of them. In many cases, interactions are intended or are managed by close monitoring. Speak to your doctor about how any drug interactions are being managed or should be managed.

Medications other than those listed above may interact with Renagel. Tell your doctor or prescriber about all prescription, over-the-counter (non-prescription), and herbal medications that you are taking. Also tell them about any supplements you take. Since caffeine, decongestants, alcohol, the nicotine from cigarettes, or street drugs can affect the action of many medications, you should let your prescriber know if you use them.
